HOW TO APPLY?
i-Medic-Id Card is available by submiting the official application form . Please view the sample and fill all the required information. Verify with your doctor for information which require medical advice.
Upon receiving your application, your Personal and Vital Emergency Medical Information will be stored in the form of i-Medic-Id Page in our server. In case an emergency occur, medical personnel will retrieve the record by accessing your i-Medic-Id Page.
We will provide you with a "wallet" i-Medic-Id Card, a laminated plastic about the size of a credit card, that contains your primary personal info and the URL address (or hotkey) for medical personnel to access your i-Medic-Id Page on the Internet.
